We accept motor vehicle/auto accident claims, worker comp claims and most major medical insurance companies. We also offer a prompt-pay discount for those paying for care without insurance and a Wellness Membership plan for patients looking for regular wellness treatments at a discounted rate.
We accept Visa, MasterCard, American Express, in-state checks with a valid ID or cash for co-pays, co-insurance, deductibles or prompt pay discount appointments. If you are considering using your insurance but you are unsure of your benefits, please take a look at our insurance link and learn how to contact your insurance company and what questions to ask.
To make your first appointment, please call our office at 503-493-9398 and select extension “0.” Tell our front desk coordinator that you are a new patient and would like to schedule an appointment. You may also email the front desk at firstname.lastname@example.org.

- Your email address
- Your date of birth

All new patients are asked to complete our online New Patient Paperwork. You will receive a link to the paperwork via email when your first appointment is scheduled. This paperwork will need to be completed prior to your first appointment and can be completed using any computer, Chromebook, tablet or cell phone from the comfort of your home.
