All new patients are asked to fill out our New Patient Paperwork Packet. You may either choose to arrive 25-30 minutes early to your appointment and complete it in our office, or you may download one of the PDFs below and fill it out in advance. Either way, please allow ample time to complete the necessary paperwork. The packet is quite thorough, as for your convenience, it covers health information and history that are applicable to all services at our center.

Use this paperwork if you are only being seen for massage and are paying at the time of your appointment. Massage patients who are using their insurance benefits or who also expect to see an acupuncturist or chiropractor should fill out the “General” paperwork above instead.
